The second session in the series will be on "

Overview:

  1. Barrier Failure Analysis (BFA) is an incident analysis technique for determining the root cause of incidents. It is one of a class of best-practice incident analysis techniques, along with Tripod Beta and BSCAT, aimed at explaining incidents / accidents as being attributed to various degrees of failure in the organizational barriers, controls, defenses and safeguards propounded by modern incident causation theories such as the Swiss Cheese Model.
  2. Gain insights into the principles and workings of the BFA technique and the use of the IncidentXP software by attending this free online webinar.
